Leeds Teaching Hospitals Trust - The Learning Disabilities & Autism team The Learning Disabilities and Autim Team support NHS staff to understand what reasonable adjustments are and to have discussions with individuals, their carers and paid carers to identify how they can be appropriately supported whilst accessing any of their services. Hospital Passport The hospital passport is offered to everyone who is autistic and is available to all community services in Leeds. You can complete one and take it with you to share with the hospital staff if you are going into hospital for an admission or attending an outpatient appointment.
